Nurses
Practice nurses are qualified and registered nurses. They can help with health issues such as family planning, healthy living advice, blood pressure checks and dressings. The practice nurses run clinics for long-term health conditions such as asthma or diabetes, minor ailment clinics and carry out cervical smears.
Please check with reception when booking your appointment that the nurse can offer the service you require.
Healthcare Assistants
Healthcare assistants support practice nurses with their daily work and carry out tasks such as blood pressure measurement, NHS health checks and new patient checks. They may act as a chaperone when a patient or doctor requests one.

Reception
The receptionists are your link with the rest of the practice. They may need to ask you medical details in order to work effectively. All information given to them is treated in confidence. We are proud to have a great team of efficient and courteous receptionists, however please be aware that they have a very tough job trying to balance the needs of all. We therefore insist that they are in turn treated in a courteous manner. Abusive or violent behaviour towards a staff member will prompt removal from the list.

Social Prescribers
Social prescribing involves helping patients to improve their health, wellbeing and social welfare by connecting them to community services.
Recognising that people’s health and wellbeing are determined mostly by a range of social, economic and environmental factors, social prescribing seeks to address people’s needs in a holistic way. It also aims to support individuals to take greater control of their own health.
Patients who could benefit from social prescribing include people with mild or long-term mental health problems, people with complex needs, people who are socially isolated and those with multiple long-term conditions who frequently attend either primary or secondary health care. Speak to your GP about the referral.

Care Co-ordinators
Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

The role is to support patients to develop the knowledge, skills and confidence to become active participants in their own health and wellbeing. To guide patients to take more control of their own health by supporting them to develop their own plan by identifying their needs and their own goals, that can lead to long term lifestyle and behaviour change. Mental Health - GP Plus
GP Plus service is a primary care mental health service embedded in primary care networks (PCNs) to provide local and timely mental health advice and information and also brief interventions for individuals aged 18-65 with a moderate mental health problems. Speak to your GP about the referral.

Physiotherapy
Physio First is our in house physiotherapy assessment team. They hold clinics regularly in the practice at both our Potterells main site, and our University site.
The Physio First team will be able to assess your needs and either supply what you need in house, or refer you through to the hospital.
