The patient group have organised a weekly health walk starting from the Brookmans Hotel every Thursday at 10.30, see the PPG pages for further information.
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bills (x2). We also require one form of photographic ID plus your NHS No. You can present to the practice on the following days/times to register: Monday to Friday 12.00 till 2.00pm and Monday evening between 6.30pm and 8.30pm.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be found on the above "Registering Online" tab. Please allow 5 days before you come into the surgery with the relevant paperwork required. i.e. utility bill and proof of ID.
All new patients over the age of 35 are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
During the new patient health check you may be offered a routine HIV screening test. We are part of a pilot project offering routine tests to all patients with the aim of improving the rate of detection of HIV in patients without symptoms.

A Patient Guide to GP Services
Please read information on the "Registration" tab above before you complete the pre-registration form. This form does not automatically register you with the practice you still have to present in person with the information required which is covered in the "Registration" tab. If you do still wish to pre-register click on the links below to open the forms. When you have completed all of the details, click on the "Send" button to mail your form to us.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
